About Veterans Care Coordination (VCC) :

About the Role :

In this hybrid role, you will play a vital role in supporting our veteran and family clients by :

  • Building trusting relationships with veterans and their families, listening to their needs, and providing support and resources as needed.
  • Coordinating and assisting with a variety of in-home services, such transportation, personal care, and medical supplies.
  • Providing referrals to community resources and government benefits programs.
  • Maintaining accurate client records and ensuring all information is kept confidential.
  • Collaborating with the family and home care agency to ensure client receive holistic support.

Who We Are Looking For :

  • Minimum 1 year of experience in customer service, social services, or a related field.
  • Strong communication and interpersonal skills , with the ability to build rapport with people from diverse backgrounds.
  • Excellent organizational skills and the ability to manage multiple tasks simultaneously.
  • Familiarity with veteran resources and benefits is a plus.
  • Proficiency in Microsoft Office Suite or Google Workspace is required.
  • Strong work ethic, a positive attitude, and a commitment to providing exceptional service.
