Job Description
Facilitate communicationbetween the patient and the interdisciplinary team.
Develop and maintain atherapeutic relationship by providing education and promoting self-advocacy andindependence.
Utilize state-of-the-arttechnology to document skilled services provided.
Qualifications
Licensed Practical Nursewith Valid CT LPN license
One year of LPN experience;Homecare experience preferred.
Valid driver’s license andreliable transportation required.
Computer proficiencyrequired.
