Job Roles
- Provide appropriate support with information and guidance about pediatric medical and support services.
- Conduct initial and on-going assessment of clients' health and / or support service needs. Set level of client need.
- Facilitate care transitions between providers, partners, and referral sources and specialty care providers. Follow-up on patient referrals for services and engagement in referral services within established timeframes.
- Participate in case reviews to provide updates on the program caseload and general trends of families.
- Facilitate enrollment of patients in specialty services to include appointment scheduling, recalls and follow-up on missed appointments.
- Engage with patients and families in communicating the importance of medical well-child exam visits and support services visits.
- Provide basic and intensive individual support, based on client need. Support may include providing interventions and internal and community services referrals.
- Complete all required documentation accurately and thoroughly, in a timely manner, in accordance with department standards and complies with patients' insurance requirements.
- Track, monitor, and actively manage assigned patient cases to ensure coordination of care plans, retention of patient, and ensuring a high level of utilization is maintained.
- Maintain and communicate accurate information in E-HR to ensure all activities are documented and conducted in compliance with applicable regulatory requirements and accreditation standards.
- Perform other duties as assigned.
Education / Certifications / Licenses / Registrations
- Bachelor's degree in Health Science or Social Science field, or closely related field preferred.
- Medical Assistant diploma, or medical terminology certificate desirable.
- High School diploma or GED equivalent required.
- Ability and means to travel as needed in a timely manner within San Diego County, to locations that may have limited access to public transportation;
- proof of liability and property damage insurance on vehicle used is required. DRIVER REQUIREMENTS : Age 21; Licensed for a minimum of 3 years;
No violations and / or accidents within 3 years; No vehicle related suspensions / reinstatements; No DUI, reckless or felony Driving within 7 years.
Experience / Specialized skills (including Language)
- 1 year of work experience providing services to medically underserved or relevant community health populations required.
- Experience applying evidence-based behavioral change models to wellness interventions preferred.
- Knowledge of the community resources, health and social service systems in San Diego County and skill in establishing working relationships with community partners preferred.
- Basic counseling skills related to reflecting, active listening and paraphrasing.
- Flexible schedule with the ability to work on Saturdays if caseload dictates.
- Basic computer literacy to comply with department needs and expectations.
- Bilingual in Spanish / English required.
- Exceptional communication and problem solving skills.
- Good written and verbal communication skills.
- Demonstrated ability to be culturally sensitive and respect diversity.
- Excellent interpersonal and customer service skills.
- Ability to work well in both a team-based environment and independently.

If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and / or experience.
In addition to these factors - we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer.
Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
