POSITION SUMMARY

The LifeSet Specialist / Case Manager is responsible for providing services to address the needs of young people ages 17.

5-21 who are transitioning out of care and need individualized skill development and support to transition successfully into adulthood, specifically obtaining and maintaining safe and suitable housing, continuing their education, learning independent living skills, obtaining and maintaining employment, building a support system, and remaining free from legal involvement.

Works to establish / re-establish meaningful relationships with parents, siblings, or other significant adults; as appropriate, coordinates family contact.

Assures that all service requirements of the DCFS are complied with, including service plans, sibling visitation, and court hearings;

manages all aspects of discharge planning in coordination with the rest of the Team.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

The LifeSet Case Manager / Specialist provides individual services to 6-8 young adults in the home and community setting to assist the young adult with the transition to adulthood, meeting a minimum of one time per week face to face with the young person and providing additional support via phone, text, or in-person as needed.

When indicated, a LifeSet Case Manager / Specialist may see a young person daily to ensure safety and stability.

Additionally, the LifeSet Case Manager / Specialist is directly responsible for all interventions associated with a specific young adult and their family.

This includes case management and collaboration with external key stakeholders and referral sources, such as probation, child welfare workers, therapists, etc.

  • The Case Manager / Specialist conducts an ongoing assessment of the young person's needs from a strength-focused, solution-based perspective and develops service plans on a monthly basis.
  • The Case Manager / Specialist also documents all service contact with the young adult within the timelines required by the organization.
  • The Case Manager / Specialist is expected to attend all meetings related to the young adult and / or family with the purpose of gathering information specific to intervention development and advocating in the best interest of the young adult's long-term stability.

Hoyleton offers 24 / 7 on-call to meet a young person's needs after hours and on weekends and holidays. The specialist is included in an on-call rotation based on current program expectations.

  • In addition, the LifeSet Case Manager / Specialist is expected to attend and actively participate in individual and team supervision sessions and training and to follow up and / or implement feedback and recommendations received from the team supervisor, program director, and clinical consultant.
  • The Case Manager / Specialist is expected to adhere to all ethical and legal guidelines, including confidentiality and mandated reporting.
  • Develops service plans as required by the Department of Children and Family Services; attends Administrative Case Reviews, court hearings, IEP meetings, and other reviews and staffings, as required.
  • Conducts detailed review of youth's case record to identify parents, siblings, foster parents, and other significant adults from the child's present and past.

Conducts assessments of their capacity and willingness to become or remain a resource to the youth. Establishes a visiting plan and coordinates its implementation.

The Case Manager / Specialist completes and maintains case records in accordance with DCFS requirements, including face-to-face contact, phone contact, and collateral contacts.

SKILLS and ABILITIES

  • Knowledge and / or experience working with adolescents preferred.
  • Good oral and written communication skills.
  • Possess a driver's license in good standing, provide proof of insurance, and a reliable means of transportation.
  • Strong organizational skills and attention to detail.
  • Ability to maintain a flexible schedule, and to manage multiple priorities simultaneously.
  • Excellent and verbal skills

EDUCATION AND / OR EXPERIENCE

  • Master's degree in the field of social work, psychology, or related field preferred.
  • A Bachelor's degree in social services with two years of related experience in counseling and / or case management is required.
  • Licensure by DCFS or eligible for licensing.

CARE & TCI

One of the essential job functions of any position within the agency is to align your performance with our overarching philosophy of CARE.

All decisions, actions, communication, and performance should align with the six CARE principles (Developmentally Focused, Family Involved, Relationship-Based, Ecologically Oriented, Competence Centered, and Trauma-Informed).

Not only must you complete the CARE training with satisfactory participation, but you must then put that training into practice in your everyday work.

Every employee must also pass TCI training with a satisfactory grade in written and / or physical testing. All employees must remain current on refresher training that is provided monthly at all locations.

You will be tested every 6 months on the physical portion, if applicable to your position. You will be tested annually on the written portion regardless of your position.

If you are not current on your TCI refresher training every six months, you will be required to retake the full course. If you do not pass either test, you may be required to retake the full course.

Not only must you complete the TCI training with satisfactory participation, but you must then put that training into practice in your everyday work.
