First Choice Health Centers is looking for a Care Coordinator to work alongside our Behavioral Health team in our 92 CT Blvd, East Hartford location to provide comprehensive, coordinated care through team collaboration, case conferencing, and outreach.
The schedule for this position is Monday, Wednesday, Thursday, 8 : 00 am to 4 : 30 pm, Tuesday, 8 : 00 am to 6 : 00 pm and Friday, 8 : 00 am to 3 pm.

- Primarily a Monday through Friday schedule working 8 : 00 am to 4 : 30 pm. There is one late night until 6 pm but you will get out on Friday at 3 pm.
- The hourly range for this position is $17.00 - $19.50 / hour. Pay is based on several factors including but not limited to work experience, certifications, etc.

Minimum Knowledge, Skills & Abilities Required :
- CPR Certified
- Bilingual English / Spanish preferred
Experience and Training :
- A minimum of an Associate's degree with two years' experience in care coordination, health navigation or community health education (equivalent experience in lieu of education will be accepted with a minimum requirement of a HS Diploma)
- Knowledge of the Social Determinants of Health
- Must have transportation to provide onsite services in other First Choice locations

Primary Tasks :
- Facilitate referrals to the appropriate community and state services and resources Educate patients about First Choice Health Center's Behavioral Health Services, Internal Medicine, Family Medicine, Dental and other specialty providers
- Attend Community Programs to establish referral relationships and patient attribution to First Choice Health Center
- Maintains updated patient records in eCW related to care coordination activities or the designated electronic medical record.
- Timely outreach to patients upon referral to the Care Coordination Program Able to prioritize and manage patient case load.
- Adhere to all patient safety standards and infection control guidelines.
