Key ResponsibilitiesConduct telehealth video visits and a limited number of home-based visits for comprehensive assessmentsPerform comprehensive diagnosis and medication reviews, focusing on quality of care interventionsCollaborate with the member's care team to coordinate care and develop a collaborative care planRequired QualificationsAPRN with 3 or more years of outpatient clinical practice experienceActive and clear RN and APRN licensesActive BLS certification required at hire and must be maintainedExperience in primary care, geriatrics, or palliative care preferredFamiliarity with telehealth platforms and managed care concepts is a plus
