Responsibilities :

- Assist clients with mobility, transfers, and positioning while utilizing safe techniques.
- Help clients with medication reminders and perform basic health monitoring.
- Ensure a clean, safe, and comfortable environment in both home and facility settings.
- Engage in effective communication with clients, their families, and the healthcare team.
- Travel between client homes and facilities as scheduled.
