Job Title : Home Health Admission & OASIS Registered Nurse (RN)
Compensation : $200 per visit + mileage reimbursement

Location : Twin Cities Metro Area
Schedule : Full-Time or Per-Diem Availability

Position Summary :
Are you looking for your next home care career as a Registered Nurse? Home Health Care Incorporated has a fantastic opportunity for an RN to perform admission and start of care visits around the Twin Cities.

Responsibilities :
Admission / OASIS Visits :
o Perform Admission / "Start of Care" OASIS Visits throughout an area of the Metro.
Client Evaluation :
o Evaluate and assess clients for home health services and create their plan of care.
Service Recommendation :
o Recommend services from other disciplines that have not yet been ordered by the physician.
Collaboration :
o Collaborate with clients, families, physicians, and other members of the home care team.
Qualifications :
- Licensed as a Registered Nurse in the state of Minnesota.
- Driving is required; must have a reliable vehicle with current insurance and a valid Driver's License.
- Prior Home Health Nursing experience.
- Proficiency with Medicare & OASIS documentation.
- Demonstrated ability to work autonomously.
- Excellent at planning, organizing, and communicating.
