About The Position

Job Title : Hospice Registered Nurse (RN) Job Specialty : Hospice Care Job Duration : 12 months Shift : Monday to Friday, 8 AM - 5 PM (5x8 Hr) Guaranteed Hours : 40 hours per week Experience : Minimum of one year as an RN in acute care hospital, oncology, long-term care, hospice, or home health License : Current license to practice nursing in the state of employment Certifications : BLS (American Heart Association), Current automobile insurance, Valid driver’s license Must-Have : English proficiency (Speak, Read, Write) Job Description : Provide direct patient care services to hospice patients and supportive care to the patient's family.

  • Manage an interdisciplinary group consisting of Licensed Practical / Vocational Nurses, Certified Nursing Assistants, and support staff to enhance hospice care quality.
  • Assure the highest quality of care is provided and documented in accordance with the Plan of Care.- Perform initial and ongoing assessments of patients.
  • Help develop and implement the Hospice Plan of Care as prescribed by the physician and in conjunction with other Interdisciplinary Group members.
  • Work closely with other Interdisciplinary Group members to provide comprehensive physical, psychosocial, educational, emotional, and spiritual services, including bereavement support, to the patient and family.
  • Report any change in the patient’s condition to both the Attending Physician and Hospice Physician.- Maintain accurate and updated medical records, including all interdisciplinary notes, Plans of Care, initial and ongoing assessments, and other pertinent patient care information.
  • Practice infection control measures and standard precautions.- Monitor staff scheduling.- Participate in the Quality Assessment and Performance Improvement Program.
  • May be requested to participate in the agency on-call schedule.- Consistently promote the company's core values.- Complete required compliance annual training.
  • Participate in staff meetings.- Maintain a high level of knowledge concerning the management of pain and symptoms in the dying process.
  • Make routine supervisory visits to all other team members at least monthly.- Perform ongoing review and follow-up on assigned patient clinical records to assure quality services are provided, standards of practice are followed, and compliance with policies, procedures, and regulatory requirements is met and maintained.
  • Maintain primary control and professional management of each patient and act as the primary liaison between physician, patient / family, and hospice team, staying fully informed of all clinical issues, staffing, and scheduling concerns.
  • Educate the patient, family, or other caregivers concerning needs identified in the Plan of Care, including medication administration, dressing changes, oxygen administration, Hospice philosophy, and care of the terminally ill patient.
  • May determine and pronounce death in the absence of an attending physician.- Ensure RN availability at all times to provide or supervise nursing care provision.
  • Supervise aides and observe them delivering care before they provide care independently to patients, completing an initial competency evaluation for all tasks assigned to the aide.
  • Prepare written instructions for aide patient care consistent with the interdisciplinary plan of care and make and document supervisory visits to the terminally ill patient's residence or living facility at least every two weeks to assess aide service performance.
  • Annually provide written evidence of direct observation of aide performance, determining competency for all required job tasks.
