Hospice El Paso is the oldest and largest non-profit hospice organization in the community. We were founded by El Pasoans for the sole purpose of providing specialized care, comfort and support to persons with a terminal illness, regardless of their ability to pay.

Our main focus is maximizing the remaining quality of life for the dying, while also providing support for their caregivers and loved ones.

Specialized teams provide compassionate comfort and care and are comprised of physicians, registered nurses (RNs), certified nursing assistants, social workers, chaplains;

plus admissions counselors and support staff.

The majority of these services are provided at home. Hospice care can also be provided in nursing, foster, and assisted living homes;

along with rehabilitation centers and skilled nursing facilities.

The Center for Compassionate Care is the city's first stand-alone hospice in-patient facility. We provide acute hospice care in a quiet and comfortable setting.

In this home-like setting, patients receive 24-hours of specialized skilled care when pain and symptoms cannot be managed in a home environment.

PURPOSE OF POSITION

To provide the highest possible standard of specialized holistic nursing care as prescribed by the physician in compliance with applicable LVN licensure and agency policies and procedures to patients admitted to Hospice El Paso’s Center for Compassion Care.

  • Assessing, planning, implementing, and evaluating individual care after admission to discharge or death.
  • Performs intermittent physical, psycho-social, and spiritual assessment after the start of care.
  • Works with Certified Nursing Assistants to ensure that the highest standard of patient care is provided and will complete CNA duties as needed.
  • Assess and document current pain level and intervention outcomes with every contact and PRN.
  • Instructs patient to overcome identified knowledge deficits and documents all instructions given.
  • Reports changes in the patient’s condition to the RN, DCO, and physician as appropriate.
  • Obtains physician orders and implements interventions and changes in treatment as appropriate. Documents all phone contact by the end of the shift.
  • Assesses and evaluates medication levels at each shift, ensuring the patient always has sufficient quantity, to adjust levels to coordinate with the patient’s current status.
  • Maintains accuracy in recording, maintenance, and administration of medications in accordance to policy.
  • Initiates prescribed treatment and evaluated medication effectiveness as designated in the POC within 1hour of intervention.
  • Makes appropriate referrals to other disciplines for family / caregiver support.
  • Works with and communicates with IDG members for admissions, transfers, discharges and changes in the POC.

Requirements :

  • Administration
  • Revises POC PRN with changes in level of care, medication, and treatment, and informs all IDG members of all changes. Ensures care plan matches contact notes.
  • Provide appropriate and accurate documentation of all skilled nursing assessments and interventions before the end of the assigned shift.
  • Provides instruction, leadership, and direction to CNAs.
  • Respond to the patient's call bells promptly.
  • Clear all chart deficiencies every week with HIMS.
  • Transcribes orders from physicians and updates medications and records by the end of assigned shift. Informing all necessary team members of updates.

Documenting all communication.

  • Prepares weekly patient care conference reports. Has a report available 1 day before the care conference.
  • Provides accurate bedside report to oncoming staff of patient’s condition and changes in the plan of care.
  • Revises CNA care plan as needed.
  • Covers extra shifts as needed.
  • Complies with agency policies and procedures, mission and values statement.
