Responsibilities :
Perform clinical assessment andidentify patient needs.
Documentprepare and complete patient Plan of Care. Track progress andrevise as needed.
Meet mandatorycontinuing educationrequirements.
Plan and provideappropriate treatment for neuromuscular neurological and orthopedicdisorders based on medical referral and therapyassessment.
Assist and instructpatients and families in transfer techniques appropriate to patientabilities. Instruct patients and families on the use of equipmentsuch as walkers wheelchairs canes etc.
in a safe manner. Utilizeappropriate therapy equipment specific to patient diagnosis andneeds.
Promote company missionand administrative policies to ensure the quality ofcare.
Requirements
Bachelor of Science in Physical Therapy from an American PhysicalTherapy Associationapprovedprogram
Current Texas StateLicense as a Physical Therapist
Current Texas Driver s License and reliabletransportation
Six months ofexperience as a Physical Therapist in an acute care orrehabilitation setting. Pediatric experience ispreferred.
Effectiveinterpersonal relations and communicationskills
Knowledge of durablemedical equipment
Ability to worka flexible schedule and travellocally
Able to work in apatients home in variousconditions
Excellentorganizational and time managementskills
Proficient in MicrosoftOffice
Able to maintain a highlevel of confidentiality
Proof ofcurrent CPR certification
Ability to do heavy lifting bending pulling pushing and standing.Prolonged standing and walking arerequired.
Requires working undersome stressful conditions to meet deadlines and patient needs andto meet patient / family individualized psychosocialneeds.
