We will consider candidates with their Clinical Social Work Associate license working towards a Licensed Clinical Social Worker license for a Clinical Social Worker I position ($38.

50 min - $55.06 max) or candidates with their Licensed Clinical Social Worker license for a Clinical Social Worker II position ($40.43 min - $57.81 max).

Responsibilities

Provides key social work services in a variety of pediatric settings across the health care continuum. Key services include (but are not limited to) : psychosocial assessments, crisis management, planning and coordination of internal and external services, resource connection, transition / discharge planning, supportive counseling, psychosocial education, post-discharge follow-up, and in limited settings, concurrent utilization review.

This position is intended for use in smaller operating units, as part of a resource pool, or for occasions that call for sporadic or stand-by work scheduling only.

Qualifications

Clinical Social Worker I CSWA position requirements :

Education :

Master of Social Work degree from an accredited School of Social Work required.

Experience :

Minimum one year post-Masters experience in one or more of the following areas required : Healthcare, discharge planning, case management or coordination of home based services, or has previously completed a Legacy Social Work internship.

Pediatric medical experience strongly preferred.

Clinical Social Worker II LCSW position requirements :

Education :

Master of Social Work degree from an accredited School of Social Work required.

Experience :

Two years post-Masters experience in one or more of the following areas required : Healthcare, discharge planning, case management or coordination of home based services, or has previously completed a Legacy Social Work internship.

Pediatric medical experience strongly preferred.

Skill Requirements :

• Broad knowledge of the principles of social work and the ability to apply them
• Demonstrates strong organizational skills, including an ability to effectively triage patient and family needs
• Practices with strengths based practice focus
• Engages in culturally competent practice
• Demonstrates excellent verbal and written communication skills for effective and appropriate interactions with patients, families, physicians and staff
• Effectively engages in discharge / transition planning
• Demonstrates ability to locate, utilize and advocate for patient / family access to hospital / community resources
• Exhibits knowledge of legal / regulatory issues and an ability to be a resource to all team members regarding such issues

Licensure

Clinical Social Worker I CSWA position requirements :

Clinical Social Work Associate (CSWA, working toward LCSW) required in OR.

Clinical Social Worker II LCSW position requirements :

Licensed Clinical Social Worker (LCSW) in OR.

• LCSW eligible strongly preferred for employees who perform this job in the state of Washington.
• Washington Counselor registration required for state of Washington employees who do not currently have a LCSW.

Pay Range

USD $38.50 - USD $57.81 / Hr.

Working Conditions
