This is a highly independent and professional position in the Community Health Education and Nutrition (CHEN) Division. Responsible for a variety of activities related to community health assessment, planning, implementation, and evaluation. The position is expected to work collaboratively with multiple community partners. Serves as the tertiary Public Information Officer and will work collaboratively on special projects with other divisions within Escambia County Health Department (Escambia CHD). All work is carried out under the supervision of the Asst County Health Dept Director-HLTH. Travel may be required.
COMMUNITY HEALTH ASSESSMENT AND PLANNING
Works collaboratively with community partners to prepare a community health assessment and improvement plan. Provides technical assistance to community workgroups completing Community Health Improvement Plan (CHIP) action plans. Completes quarterly progress updates and an annual report. Ensures the assessment and plan follows the MAPP process and meets evidence-based standards established by the Public Health Accreditation Board. Submits required Community Health Assessment (CHA) and CHIP documentations to the state office within established timelines. Plans and implements the collection and analysis of primary data that will inform community and agency health improvement plans and the strategic planning process. Identifies and recommends updates to plans and activities based on new data.
REACCREDITATION
Leads the reaccreditation process for the domains concerning community health assessment and improvement planning, implementation, and evaluation. Serves as primary point of contact with central office reaccreditation team on all matters related to the reaccreditation process, documentation, etc. Obtains information from community partners as required by PHAB. Participates in the quarterly regional PMQI meetings.
COMMUNITY ENGAGEMENT
Prepares and submits reports for statewide initiatives. Identifies, initiates, and strengthens relationships with various community stakeholders; Implements meetings, surveys, and other means of communication with the community on Escambia CHD initiatives. Attends community meetings on behalf of Escambia CHD as a community liaison. Maintains various databases of groups of stakeholders; Creates internal and external evaluation of engagement initiatives; Distributes information to the community and broader public through multiple channels, such as web, social media, etc.
PROGRAM EVALUATION
Ensures that division health promotion initiatives include an evaluation component. Documents outcomes in an annual report. Coordinates data collection and evaluation for initiatives in all sectors. Documents outcomes in the strategic plan annual report. Plans evaluation component of annual core campaigns including prevention messages for flu, mosquito borne illnesses, drowning, and sun exposure. Provides technical assistance to other divisions within Escambia CHD in the planning, implementation and evaluation of public health marketing campaigns, interventions, or data queries.
LIFELONG LEARNING
Participates in required training and continuing education activities. Maintains active Community Health Education Specialist certification through the gathering of continuing education hours.
