Department : Memory Care
Work Hours : Full-time Monday through Friday, with availability for off-shift and weekend coverage
This position leads and oversees the day-to-day operation of our Memory Care Neighborhood and Quality Assurance efforts campus-wide.
This role works in concert with other departments, outside agencies, families and physicians to ensure coordinated resident care delivery, and an optimal environment for resident in need of memory care programing.
Helps to ensure the goal of quality resident care is to enable each resident, where possible, to return to the highest level of functioning, to attain a new stage of independence, or to at least prevent regression.
This is a hands-on leadership position that is expected to support the mission, vision, and goals of Heritage Valley Health System and Heritage Valley Senior Living Community by upholding and promoting company culture and vision.
The Quality Assurance Nurse Specialist will be part of the nursing administration team requiring staff supervision and on call responsibilities.
Requirements :
Must have an active Pennsylvania LPN or RN License. At least 5 years of personal care or long term care experience. At least 1 year of Dementia Specific Care Leadership.
Must be able to read, write, and speak English. Must possess the ability to follow oral and written direction.
Preferred : Certified Dementia Practitioner
