Job Title : Family Nurse Practitioner Primary Care
Job Type : Full-Time
Position Overview : As a Family Nurse Practitioner, you will play a vital role in delivering comprehensive primary care services to patients of all ages.
You will work collaboratively with physicians, nurses, and other healthcare professionals to provide holistic care, emphasizing health promotion, disease prevention, and chronic disease management.
This full-time role is ideal for practitioners who are kind, caring, and committed to building lasting relationships with their patients.
Key Responsibilities :
- Patient Care : Conduct thorough physical exams, diagnose and treat acute and chronic illnesses, and develop personalized care plans for patients.
- Collaboration : Work closely with a multidisciplinary team to coordinate patient care and ensure optimal health outcomes.
- Patient Education : Provide education and counseling to patients and their families on health maintenance, disease prevention, and treatment options.
- Chronic Disease Management : Manage chronic conditions such as diabetes, hypertension, and asthma, focusing on improving patient outcomes and quality of life.
- Health Promotion : Advocate for and implement health promotion and disease prevention strategies in the community.
- Documentation : Maintain accurate and comprehensive electronic health records (EHR) in compliance with healthcare regulations and standards.
- Follow-Up : Monitor patient progress, adjust treatment plans as necessary, and provide continuous support to patients throughout their healthcare journey.
Required Skills and Certifications :
- Board Certification : Board Certification as a Family Nurse Practitioner is required.
- NYS License : New York State (NYS) License to practice as a Family Nurse Practitioner is required.
- Experience : Previous experience in a primary care setting is preferred, with a focus on delivering patient-centered care.
- Skills : Excellent clinical, communication, and interpersonal skills, with a strong commitment to patient care and safety.
