• Job Summary : *A PRN Physical Therapist who provides coordinated physical therapy to home health patients. We offer competitive pay and significant opportunities for advancement within the company!

Responsibilities Include :

• Perform initial and ongoing clinical assessments to identify patient needs, determine level of functioning, including OASIS assessments at appropriate time points and appropriateness of patient for home health.
• Develop and revise a Plan of Care in consultation with the physician and other care-team members.
• Provide therapeutic treatments and evaluation of equipment needs as applicable.
• Evaluate outcomes of care with appropriate discharge planning.
• Apply concepts of infection control and standard precautions in coordinating / performing patient care activities to prevent contamination and transmission of disease.
• Accept clinical assignments that are consistent with education and competence to care for patients.
• Supervise PTA and HHA as appropriate.
• Complete appropriate documentation in a timely manner.
• Meet mandatory continuing education requirements of the agency / licensing board and participate in in-service programs.
• Use effective interpersonal relations and communication skills to advise and consult with the family and other agency personnel as appropriate.
• Promote company philosophy and administrative policies.

Job Qualifications :

• Education : Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program.
• Licensure : Current Texas State License as a Physical Therapist, current Texas Driver’s License
• Skills : Therapy skills as defined as generally accepted standards of practice. Good interpersonal skills. Proof of current CPR.

Knowledge of durable medical equipment.

• Transportation : Reliable transportation and valid and current auto liability insurance.
• Experience : 1-2+ years of Home Health experience and Manual Therapy experience are preferred.

Pay : $85,000.00 - $125,000.00 per year
