Key Responsibilities : Build and maintain strong relationships with patients, their families, and facility staff, fostering trust and collaboration.

Provide compassionate care to assisted living patients, addressing their physical, emotional, and social needs. Communicate effectively and empathetically with patients and their families to ensure understanding of treatment plans and care goals.

Collaborate closely with facility staff to ensure seamless care and support for patients. Maintain a flexible schedule, balancing patient visits with some in-office hours (ideally every other Friday from 9 AM - 1 PM).

Apply hospice or home health experience to deliver exceptional care to patients requiring complex medical attention. Youre a Great Fit for This Role If You Are : You are passionate about forming deep, trusting relationships with patients and their families.

Requirements Qualifications : Licensed Nurse Practitioner or Physician A ssistant with 3 years clinical experience, preferably in home health or hospice care.

Masters degree from an AMA accredited school. Active, non-probationary, unrestricted Georgia State PA or NP License Active unencumbered DEA License for Georgia OR eligibility to obtain Ability to work in North Metro Atlanta area (Duluth office / facilities in Woodstock, Cumming,, Canton).
