LPN / RN Nurse for Hospice\n\nAlternative Hospice is seeking a passionate LPN / RN to join our growing team. As an LPN or RN , you would be able to work independently, be dependable, compassionate and have a desire to help others. The Licensed Practical Nurse ( LPN ) or Registered Nurse ( RN ) will travel to patients' homes and be responsible for administering individualized patient care and perform duties related to hospice and palliative care in accordance with physician orders.\n\nLPN / RN Nurse Responsibilities\n

• Demonstrate knowledge of personal hygiene techniques.\n
• Work with a team and implement nursing care plan for each patient.\n
• Communicate effectively with patients, families, physicians, and other healthcare providers.\n
• LPN / RN provides physical care to patients in a home- or community-based setting.\n
• Assess patient's physical and mental status; initiate preventative and rehabilitative nursing procedures as appropriate.\n
• Observes signs and symptoms and reports to the physician and RN reactions to treatments, including drugs and changes in the patient's physical or emotional condition.\n
• Teaches and counsels the patient and family / significant others regarding the nursing care needs and other related problems of the patient at home.\n
• LPN / RN maintains accurate and complete records of observations, treatments and care of patient.\n
• Takes on-call duty rotation; week-day night coverage when needed to fill voids, two weekends a month, and one holiday a year.\n
