Job Title : Rehabilitation Therapy - Physical Therapy Assistant Job Specialty : Physical Therapy Assistant Job Duration : 13 weeks Shift : Monday - Friday, Days Guaranteed Hours : 32 hours per week Experience : SNF experience highly preferred;
Rehab Optima experience preferred License : State License Verification required Certifications : None specified Must-Have : Completed prescreen attached upon candidate submission- Resume and license verification attached - Background checks and searches as listed below Job Description : Provide support under the supervision of a licensed physical therapist.
- Assist in providing therapy services to patients and help in evaluating progress.- Document patient treatments and progress accurately and promptly.
- Communicate effectively with patients, families, and team members.- Maintain a clean and organized work environment.- Participate in team meetings and collaborate effectively with other healthcare professionals.
- Follow facility policies and procedures to ensure patient safety and quality care. Credentialing Checklist : Resume- References (3)- County Criminal Background Check (7 years for all counties lived and worked)- Federal Criminal Background Check (7 years or more)- Multi-State Criminal Search (7 years or more)- Nationwide Sex Offender Search (7 years or more)- HHS / OIG Search- GSA Search- Social Security Number Trace- 2 Step PPD or Quantiferon or Proof of Positive PPD and Chest X-Ray- SAM Search- Washington State Background Check with Fingerprints (facility-run)
