Our company is a leading provider of advanced illness management and palliative care. We are looking for a dedicated and compassionate Palliative Care Nurse Practitioner to join our team in providing personalized care to patients.

Responsibilities :

  • Work collaboratively with physicians and other team members to deliver advanced illness management and palliative care
  • Assess patients' needs and develop individualized treatment plans
  • Prescribe medications and order diagnostic tests as necessary
  • Communicate with the medical team to ensure the effectiveness of care plans

Requirements :

  • Master's degree in Nursing
  • Minimum of two years of Nurse Practitioner experience
  • Current Nurse Practitioner certification / license
  • Strong communication and leadership skills
