CARE TEAM NURSE (RN / LPN)
Location : West Salem Clinic, 1233 Edgewater Street NW, Salem OR 97304
Job Status : Full-time, Monday-Friday with rotating Saturdays
Sign-Bonus : $3,000 (Internal candidates that are currently employed here at NWHS are not eligible for a sign-on bonus.)
Referral Bonus : $1000 (Referral bonus’ expire at the close of this job posting.)
YOUR ROLE :
As our Care Team Nurse, you will use a trauma-informed and whole-person approach to address patient wellness, acute illness, chronic disease, disability, and end-of-life needs in collaboration with their assigned primary care provider and care team.
- In this role, you will educate and motivate patients in areas related to self-care; dispense telephone advice to patients with minor acute problems;
- support the ongoing care management of patients with stable chronic disease; monitor patients during routine office visits;
and perform of a variety of procedures that are within the scope of the professional nursing practice.
QUALIFICATIONS :
- Oregon Nursing License (RN or LPN)
- CPR / BLS certification
- Possess clinical expertise in an outpatient environment and triage.
- Ability to exercise independent judgement and decision-making.
- Ability to prioritize competing responsibilities while under pressure.
- Demonstrates problem solving skills.
- Ability to manage high volumes of patients in a short span of time, while dealing with unpredictable issues.
- Ability to work with diverse cultures and lifestyles.
- Bilingual English / Spanish preferred.
