This position works with a collaborative physician. The Nurse Practitioner and or Physician Assistant provides primary and secondary health services for individuals and families presenting to The Palmetto Palace Mobile Health Unit for preventive services to include well-woman visits, chronic care management, immunizations, routine lab tests, risk assessment and adult physicals.

Performs other duties as assigned.

ESSENTIAL FUNCTIONS

The essential functions of this position include but are not limited to the following duties :

Provides quality medical care services to clients in accordance with generally accepted standards of professional practice and the policies of the mobile health unit.

Reviews all medical histories of clients upon initial visit to the mobile health unit and thereafter as appropriate.

Coordinates onsite medical care.

Participates in ongoing comprehensive treatment planning and review.

When appropriate, will provide onsite medical interventions.

Ensures prompt follow-up action on clients with abnormal test results or physical findings.

Reviews rehabilitation treatment plans, recommendations and orders such treatment.

Refers clients to a health care facility or health care practitioner for service(s) not available at the mobile health unit.

Participates in the mobile health units performance improvement program that includes a planned and systematic process for monitoring and assessing the quality and appropriateness of patient care and clinical performance.

Knowledgeable about policies / programs in Public Health and Medical Home that may affect patient care programs.

Available to participate in mobile units in-service training programs and for consultation regarding the mobile health unit ability to meet appropriate needs of patients families and personnel.

Available for consultation with partnered care coordination staff (s) as needed.

Completes accurate and timely submission of all clinical and professional records in the EMR to assure compliance with local, state, federal regulatory, licensure and accreditation requirements.

Work to create a positive work culture with cross departmental teams and align this work with other health care initiatives.

Monitors clinical outcomes, research best practice, and implements approved changes to address areas of concern.

Maintains clinical competency in nursing in general and in area of advanced practice nursing specialty in particular, including maintenance of national board certification and state license to practice and prescribe within defined scope.

Utilizes established channels of communication and supports team building efforts.

Participates in quarterly performance improvement committee meetings.

Maintain a professional attitude and appearance.

Recognizes, accepts and respect people as individuals.

Demonstrates leadership in advancing clinical knowledge in the area of specialty in a culturally competent manner.

COMPETENCIES

State Minimum Requirements : Positions in this classification require licensure as a Registered Nurse and certification as a Nurse Practitioner by the South Carolina Board of Nursing.

Graduation from an accredited school of nursing with clinical experience.

Physician Assistant with clinical experience in preventive care, patient education, chronic disease, and acute illness management.

Institutions of Higher Learning must be recognized by the Council for Higher Education Accreditation.

The Palmetto Palace Additional Requirements

A masters degree in nursing and license in good standing as a prescribing Advanced Practice Registered Nurse (NP) in Women's Health, Adult Health, or Family Practice.

Nationally board certified in specialty area and licensed or eligible for licensure as a prescribing NP in South Carolina.

Skilled in interviewing, history taking, and conducting health assessments.

Able to identify deviations from normal and make appropriate clinical decisions. Skilled in education and counseling techniques.

Aware of and able to partner with pertinent community resources. Able to collaborate with other health professionals and to serve as a role model / preceptor for nursing staff and student Nurse Practitioners.

Able to evaluate quality of clinical practice and implement change. Able to communicate effectively both orally and in writing.

Current driver's license; may be required to use personal vehicle in the performance of duties.

Provider CPR certification within first 6 months of employment, with renewal as required.

Possess effective oral and written communication skills and customer service skills.

Good computer literacy skills with the ability to process automated health records accurately.

WORK ENVIRONMENT

This job operates in a mobile health unit. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

POSITION TYPE / EXPECTED HOURS

This is a part time position with the requirement that the employee work during clinic service hours. On rare occasions and for special projects, evening or weekend work may be required as part of the job duties of the work.

PHYSICAL DEMANDS

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand;

walk; use hands to finger, handle or feel; and reach with hands and arms. The employee must frequently lift and / or move up to 10 pounds and occasionally lift and / or move up to 25 pounds.

Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.

Health and Wellness

Availability

Availability on a weekend Saturday, 8am-3pm 1st Saturday, or a 3rd Saturday or a 4th Sunday, 1pm-5pm
