Swing Case Manager
ONSITE ) Santa Ana, CA 92704
Schedule : (Swing) Tuesday - Saturday 3 : 30p 11 : 30p
Pay : $23 per hour
Key Requirements :
- Provide comprehensive case management services to underserved individuals.
- Conduct intakes, and assessments to determine participants needs and develop individualized service plans (ISP), schedule interdisciplinary meetings.
- Connect participants with appropriate resources and services to address barriers.
- Collaborate with community partners, shelters, and other agencies to facilitate access to resources and coordinate appropriate care.
- Maintain accurate and up-to-date documentation of participant interactions, progress, resources and referrals.
- Advocate on behalf of participants to ensure access to essential services and support their goals for housing stability and self-sufficiency.
- Participate in team meetings, case conferences, and ongoing training sessions as required.
Qualifications :
- Minimum 1 year experience in Case Management, Social Work.
- Comfortable working with homeless population.
- Strong interpersonal skills and the ability to build rapport with diverse populations.
- Excellent communication and problem-solving abilities.
- Compassionate and non-judgmental approach to working with clients.
