Summary :
This position is primarily responsible for the management & coordination of all clinical operations of Home Care. This person develops, coordinates, and manages the work performed by clinical staff (skilled nursing & therapy services).
This person creates and develops referral sources through specific skilled niches. This person coordinates education, documentation, and quality assurance in clinical operations and in patient care for home care.
This person works in cooperation with the Executive Director of Hospice to insure the clinical and financial success of the region.
Qualifications and Experience
Licensed professional RN in operating state
A minimum of two-year experience in Home Health, Hospice, or related health care field
A minimum of two-year experience in management
Excellent verbal and written communication skills and strong interpersonal skills
Ability to work in teams.
Excellent knowledge of OASIS and Medicare guidelines
Thoroughly understands financial implications of OASIS documentation.
A criminal background check must be performed before hire.
Complete and pass skills evaluation.
Essential Duties and Responsibilities :
Actively seek a knowledge base in areas managed.
Develop knowledge of agency policies, regulations, and procedures.
Knowledge of common safety hazards and precautions to establish a safe work environment.
Prepare and maintain written records, reports, and responds to correspondence.
Must have skills in developing and maintaining department quality assurance.
Must have skill in establishing and maintaining effective working relationships with agency staff and the public.
Ability to maintain quality control standards and the ability to react calmly and effectively in emergency situations.
Ability to interpret, adapt and apply guidelines and procedures.
Basic understanding of accounting and knowledge of governmental regulations
Good communication skills.
Assist Executive Director in hiring of all clinical staff and assignment of position, completing Personal Action Forms, and in potential policy changes.
Builds culture of discipline and education
Will designate an RN to act as Director of Nursing during his / her absence, and oversight may be provided by the clinical support services team.
Review and oversight of clinical procedures, policies, job descriptions for effectiveness, and will make recommendations.
Clinically analyzes Medicare Certs
Clinical education of case managers
Attends to the clinical needs of the employees.
Conduct and maintain records of periodic performance evaluations of clinical staff, salary / wage changes, disciplinary action, etc.
Assist Executive Director in conducting staff meetings, providing in-service education, development, and record keeping.
Development, oversight, and maintenance of clinical compliance issues, records, and tracking processes; oversight of chart auditing.
Oversees clinical paperwork.
Oversight, training, and implementation of the respective Federal, State, Local, Medicare, Medicaid regulations.
Report problems, employee complaints / issues, proposed goals / solutions, etc. to Executive Director.
Develop and maintain an ongoing quality improvement program for clinical staff, patient care, and educational in-services.
Develop and maintain effective patient / family confidentiality.
Assess and assure appropriate supervisory visits by nursing.
Assist in Patient Satisfaction Surveys.
Assess and assure clinical staff adherence to agency policies and procedures.
Assess and assure compliance with Universal Body Fluid Precautions and with OSHA guidelines.
Develop and facilitate the on-call nursing schedules, also participating in on-call.
Attend all regional staff meetings.
Oversee weekly case conference or IDG team meetings.
Maintain patient confidentiality.
Implement any new policies from governing board.
Evaluate physician orders for timeline accuracy and consistency with assessment and treatment plan.
Travel is required.
