Overview of the Role :
The Extensivist is a licensed Physician who provides services to the senior patient population across multiple settings.
S / he will see, treat, and manage patients primarily in the skilled nursing facility setting and in patient’s residences.
S / he will also see patients occasionally in the hospital for patient engagement.
Responsibilities :
- See, treat, and co-manage patients (high risk, frail population) in skilled nursing facilities and home setting. May also see patients in the hospital.
- Communicate daily with covering care team members and specialty teams.
- Coordinate care with community physicians and Alignment care managers to ensure smooth transitions between care settings.
- Work with other clinicians and Case Managers to ensure all patients are seen.
- Conference call with Alignment Case Managers to review patients and discuss the discharge needs and plans.
- See, treat, and manage skilled’ patients in the SNFs according to established protocols. These patients are seen at least twice a week while they remain skilled, which is normally about 2 weeks.
Patients are also seen within 72 hours of admission to the skilled nursing facility.
- Supervise, mentor, and assist Advanced Practice Clinicians (Nurse Practitioners and Physician Assistants) as needed.
- Across all settings of care, create a comprehensive and coordinated plan of care which considers information from medical records, the patient and their families, primary care physicians, and specialists as necessary :
- Medical records are to be completed in a timely fashion consistent with the standard of care.
- Complete outpatients visit documentation within 24 hours of care.
- Coordinate discharge plans with the Care Management Team
- Communicate with patients or their families / care givers and the Primary Care Physician about the plan of care and any changes in the patient’s health condition.
- Work with the Case Management Team to ensure the member is supported when care is transitioned from one setting to the next.
- Participate in weekly complex case management multidisciplinary meeting.
- Participate in on-call schedule as needed. (may vary by location.)
- Develop and communicate recommendations to AHC leadership to continuously enhance care delivery system.
- Travel locally between settings and travel to AHC corporate office on a quarterly basis or as directed.
- Perform other administrative duties as deemed necessary.
Requirements :
- 2-3 years of Hospitalist, Family Practice, or Internist experience preferred but will also consider graduating residents and fellows.
- Medical Doctorate (M.D.) or Doctor of Osteopathic Medicine (D.O.); Internal Medicine or Family Medicine Residency
- Medical License in the state in which you are applying; DEA license, must be board certified or board eligible in specialty.
- Career interest in management preferred.
- Spanish speaking preferred.
