The Case Manager RN isresponsible and accountable for coordination of patient servicesthrough an interdisciplinary process, which provides a clinical andpsychosocial approach through the continuum of care.

Throughconcurrent case management, patients will be assessed to determineappropriateness of admission, continued hospitalization, as well asappropriate level of care.

Discharge planning will begin at thetime of (or prior to) admission, and reassessed ongoing throughoutthe course of hospitalization.

Quality and Risk Managementissues will also be monitored for and reported asappropriate.

Location : Boulder Community Health- 4747 Arapahoe Ave, Boulder, CO

Schedule : 5 shifts in a pay periodwith one day being a weekend day

Primary Responsibilities :

  • Directly communicates with staff, physicians,patients and families
  • Coordinates patient careprocesses to achieve desired quality outcomes andidentifies / controls inappropriate resource utilization
  • Facilitates patient and family education and promotescontinuity of care to achieve optimal patient outcomes. Assures patient rights by offering a choice whenappropriate
  • Reviews the patient plan of carewith the multi-disciplinary team. Facilitates andparticipates in multi-disciplinary team care conferences forpatients with complex problems.

Communicates in the medicalrecord and verbally with the team to coordinate interventions andfacilitate continuity of care

Dailycommunication and collaboration with the patient care staff toprovide continuous assessment, evaluation and continuum planning toassure the patient receives the appropriate level of care at theappropriate time.

Facilitates the implementation of nursinginterventions as indicated by the multi-disciplinary team plan ofcare that enhances and compliments the skill level of the nursingstaff

  • Functions without direct supervision,utilizing time constructively and organizing assignments formaximum productivity. Arranges schedule to facilitate meetingwith physicians for patient care rounds, team meetings and otheropportunities to improve communication
  • Aninitial assessment is required to be completed for all inpatientswithin 36 hours of admission (Per employee average caseload of 12patients per day)
  • Excess Days per Patientdischarge should remain under 1.39 days per contractrequirements
  • A discharge summary is requiredfor all patient discharges
  • IndividualExpectation : Employees are to document a daily intervention on allpatients
  • Operation Expectation : Employees arerequired to lead daily Interdisciplinary Rounds for their patientson a specific medical unit to drive timely discharges and plans ofcare
  • CMS Compliance : All Medicare and MedicareAdvantage patients are required a delivery notice for MOON &IMM documentation.

All patients are also required to be providedwith choice for discharge dispositions

RequiredQualifications :

  • Associate’s Degree (or higher)
  • Valid, unrestricted RN License in the state of ColoradoOR Master of Social Work (MSW) Degree
  • Ability to work 5 shifts per pay period with one daybeing a weekend day

Preferred Qualifications :

  • Bachelor of Science in Nursing(BSN)
  • Case ManagementCertification
  • 3+ years of recentclinical or case management experience in area ofspecialization
  • Knowledge / understandingof community resources, policies, and procedures

Soft Skills :

Solid analytical, critical thinking andorganizational skills

Colorado ResidentsOnly : The salary range for Colorado residentsis $58,300 to $114,300 per year. Pay is based onseveral factors including but not limited to local labor markets,education, work experience, certifications, etc.
