GREIVANCE / CHRONIC CARE NURSE POSITION SUMMARY
The Grievance / Chronic Care nurse monitors patients from intake to the facility and ensures continuity of care leading up to and f ollowing release into the community by advocating for patient needs while acting as a liaison between the multiple systems impacting the lives of offenders to provide a proactive and collaborative approach in serving the patient population in a correctional environment.
GREIVANCE / CHRONIC CARE NURSE MINIMUM REQUIREMENTS
- Graduate of an accredited School of Nursing
- Licensure as a LPN in the state of employment.
- Possesses an active CPR certification.
- Remains knowledgeable about specific state laws and regulations governing practice.
