As a BHH Care Coordinator, you will enjoy a high level of autonomy and flexibility in managing your workload. You will coordinate and provide comprehensive care that integrates both physical and mental health, working closely with clients to understand and address their behavioral health needs, physical health needs, and other psychosocial factors.

Clinical supervision is available for LMSW-cc and current MSW students, and in-house training is provided to meet certification requirements.

Key Responsibilities :

  • Conduct psychosocial assessments to identify client needs and strengths
  • Develop and implement strengths-based treatment plans tailored to each client
  • Coordinate care with other healthcare providers and community resources
  • Promote health and wellness through education and support
  • Advocate for clients' rights and support their families in accessing necessary services
  • Facilitate small group sessions to provide additional support to clients
  • Maintain accurate and timely records of client interactions and progress
