Seeking full time MD / DO (FM / IM / Ped) to serve as a member of our Executive Leadership team as Medical Director.
The role involves serving as a primary care provider in a Patient Centered Medical Home for 50% of their workload and providing supervision and medical leadership for the medical provider team and Executive team 50% of the time.
Clinic is recognized by NCQA as a Patient Centered Medical Home with Behavioral Health Distinction, in all three clinical care delivery locations.
We practice team-based care through which we provide clinical support and holistic care by providing cradle to grave primary care services, pediatric services, prenatal care and womens health, Title X services, Behavioral Health, Dental, pharmacy with medication discount programs, community resource coordination, transportation support, and more.
