Job Summary :
ATN Healthcare is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Home Health team. The LPN will provide quality nursing care to a home health client, ensuring their health, safety, and well-being.
The ideal candidate will have experience in home health care and a passion for improving the lives of patients.
Responsibilities :
- Patient Care : Provide direct nursing care to the client according to the care plan developed by the registered nurse and physician.
- Assessment : Conduct regular assessments of the client's health status and report any changes to the healthcare team.
- Medication Management : Administer medications as prescribed and monitor for side effects or adverse reactions.
- Wound Care : Perform wound care and dressings as needed.
- Dressing
- Transfer
- Mobility
- Vital Signs Monitoring : Monitor and record vital signs, including blood pressure, pulse, respiration, and temperature.
- Documentation : Maintain accurate and detailed records of patient care, observations, and treatments.
- Education : Educate the client and their family on healthcare practices, self-care techniques, and disease management.
- Communication : Communicate effectively with the healthcare team, client, and family members to ensure coordinated and continuous care.
- Compliance : Adhere to all federal, state, and local regulations and guidelines, as well as the policies and procedures of ATN Healthcare.
- Supervise CNAs, Home Health Aids, and care givers.
Qualifications :
- Licensure : Current and valid LPN license in the state of Colorado.
- Education : Graduation from an accredited LPN program.
- Experience : Minimum of one year of experience in home health care preferred.
- Skills :
- Strong clinical and assessment skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Proficient in electronic health records (EHR) and documentation.
- Certifications : Current CPR certification.
- Transportation : Reliable transportation and valid driver's license.
Physical Requirements :
- Ability to lift and move patients as required.
- Prolonged periods of standing, walking, and moving.
- Ability to work in various home environments.
Work Schedule :
Flexible scheduling with the potential for weekends, holidays, and on-call duties as needed.
