About The Position
Job Title : Rehabilitation Therapist Job Specialty : Physical Therapist Job Duration : 13 weeks Shift : Monday - Friday, Day Shift Guaranteed Hours : 32 per week Experience : New graduates are welcome;
SNF experience preferred License : State License Verification required Certifications : COVID test required 72 hours prior to start date with results issued 48 hours before start date Must-Have : - Prescreen filled out and attached - Resume and license verification attached - Completion of credentialing checklist Job Description : - Provide comprehensive rehabilitation therapy services - Work collaboratively with patients to develop and implement individualized treatment plans - Conduct assessments and evaluate patient progress - Maintain accurate and up-to-date records - Communicate effectively with patients, families, and other healthcare professionals - Stay informed about the latest developments and techniques in physical therapy - Ensure compliance with all relevant regulations and standards Please ensure thorough completion of the credentialing checklist prior to submission, which includes : - Resume - References (3) - County Criminal background check (7 years for all counties lived and worked) - Federal Criminal background check (7 years or more) - Multi-State Criminal Search (7 years or more) - Nationwide Sex Offender Search (7 years or more) - HHS / OIG Search - GSA Search - Social Security Number Trace - 1 Step PPD or Quantiferon or Proof of positive PPD and Chest X-Ray - SAM Search
