Supportive Services Case Manager
The Supportive Services Case Manager fosters a positive and inclusive living environment for tenants with severe mental health conditions.
This role involves providing case management emotional support and ensuring effective communication between tenants, property management, and relevant stakeholders.

Responsibilities
Case Management : Conduct individual assessments to identify specific needs and challenges tenants with mental health diagnoses face.
Collaborate with mental health professionals to understand treatment plans. Utilize goal-setting and positive coping strategies to help tenants live their best life.
Community Building : Establish positive relationships within two apartment complexes. Work with each other to develop community and positive communication.
The overall goal is to decrease isolation.
Communication and Coordination : Serve as a liaison between tenants, property management, internal program staff, mental health providers, and community providers.
Communicate effectively with all stakeholders to ensure a holistic approach to tenant well-being.
Specific Duties
- Communication between property managers, program staff, community providers, and tenants.
- Individual goal setting with tenants to strengthen their independent living skills.
- Liaison with community mental health providers working with the tenants and their families.
- Resource navigation to address any barriers to accessing services, including Social Security and other county financial benefits.
- Crisis and safety planning to maintain independent living.
- Community building events and workshops for all tenants on some evenings and one weekend a month.
- Assessing and supporting aging-in-place needs to maintain independent living, being aware of the physical, developmental, and mental stages of seniors.
- Tenant relationship support to maintain harmonious living conditions.
- Data tracking and documentation for all activities related to tenants.
Qualifications
- Bachelor's degree in a related field (Social Work, Gerontology, Counseling, or similar).
- Master's degree preferred or at least five years of experience working with serious mental health conditions.
- Ability to work occasional evenings and weekends as needed.
- This position is based on-site / office.
- Proficiency in record-keeping software and Microsoft Office Suite.
- Valid CA Driver's License and reliable form of transportation required.
- Bilingual in Spanish and English is preferred but not required.
- MWC requires proof of full vaccination against COVID-19 and highly encourages staying current.
Salary
$24.00 - $31.30 per hour
