Job Description :
Oregon State Hospital has a Pharmacist vacancy! Pharmacists at OSH work collaboratively with providers to provide medical treatment to mental health patients in a hospital setting.
This position will supervise technicians, promote efficient and safe medication distribution from the central pharmacy, perform prospective drug utilization review, and work collaboratively to support clinical services including clozapine management, anticoagulation monitoring, and antibiotic stewardship.

What you will do!
You will provide professional supervision of pharmacy technicians!
You will work with a collaborative team of pharmacists, pharmacist informaticists, and inventory specialists to optimize medication distribution from the central pharmacy to 24 patient care units!
You will perform medication order review and transcription into Avatar / RxConnect platforms!
You will support clinical functions including :
Anticoagulation management and direct patient care
Clozapine management
Diabetes management
Antibiotic stewardship
Transition of care review and reconciliation

WHAT WE ARE LOOKING FOR :
Minimum Qualifications
Possess a current Pharmacist's license issued by the Oregon State Board of Pharmacy.
Special Qualifications
Possess a Decorate of Pharmacy (PharmD) from an accredited School of Pharmacy.
Preferred Qualifications
Completed a PGY-1 and PGY-2 residency or other advanced training / degree.
At least 3 years’ experience in an inpatient hospital pharmacy setting.
Experience with mental health patients and treatment
Experience with precepting pharmacy students / residents
Knowledge of Omnicell, EHR functionality
