Registered Nurse (RN) - Home Health - PRN
We have an immediate need for a licensed Registered Nurse (RN) who is seeking a PRN position in a Home Health setting for the following areas :
- North Dallas
- HIghland Park
- Irving
- Middle Dallas
QUALIFICATIONS
- Graduate of an approved school of professional nursing
- At least 1 year experience preferred
- Licensed as a registered nurse by the State of Texas or compact state
- Knowledge of general nursing theory and practice, including those basic bodies of knowledge relative to nursing, such as drugs, biological, physical, social, and medical sciences, and their applications, for better understanding of client care problems
- Thorough knowledge of principles, methods, and techniques involved in performing general nursing procedures and adapting or modifying standard nursing practices for care of specific cases
- Genuine interest in nursing
- Must have reliable transportation and valid driver's license and auto insurance coverage
JOB RESPONSIBILITIES
- Assesses needs of client, (physical etc.) including OASIS assessments at appropriate intervals
- Regularly re-evaluates the client’s needs, (physical etc.)
- Initiates the plan of treatment and establishes necessary revisions for clients receiving skilled care
- Initiates appropriate preventive and rehabilitative nursing procedures
- Prepares clinical and progress notes
- Coordinates services
- Informs the physician and other personnel of changes in the client’s condition and needs
- Counsels the client and family in meeting nursing and related needs
- Supervises and teaches LVNs, HHAs, and other professional as well as paraprofessional staff, under the direction of the Administrator
- Conducts the skilled admission of the client to the Agency by
- obtaining medical history
- conduct a brief physical
- evaluate the client's condition and environment
- review medication regimen
- Develop and implement a nurse care plan
- Provide skilled nursing to include the following :
- Nursing services, treatments and preventative care as ordered by the physician
- Initiate preventative and rehabilitative nursing procedures
- Observe signs and symptoms and reporting to the physician reactions to treatments
- Teaching, supervising and counseling the client and family members regarding the nursing care needs
- Administer medications as prescribed by the physician
- Planning for the discharge of service
- Assume responsibility for the care given by the HHA to clients in his / her care by :
- Completing an assignment sheet which specifies services to be performed by the HHA to each client per visit
- Determining need for HHA service
- Supervising and evaluating the care given by the HHA as needed at a minimum of every 14 days
- Submitting written evaluations of the HHAs who are providing services to the client
- Participating in periodic conferences concerning HHAs performance
- Regularly re-evaluates the nursing needs of the client, and participates in the development and periodic revision of the physician's plan of treatment
- Ensures availability of supplies and equipment and instruct personnel on its use
- Make supervisory visits to the client’s residence at least once every 30 days, either when the LVN is present, to observe and assist, or when the LVN is absent, to assess relationships and determine whether goals are being met.
- Prepares and presents the client's record for the Utilization Review at planned intervals
- Coordinates total client care
- Participates in client case conferences as appropriate
- Participates in the planning, operation, and evaluation of the nursing service
