About The Position
Job Title : Rehabilitation Therapist Job Specialty : Physical Therapist Job Duration : 13 weeks Shift : Monday-Friday, Days Guaranteed Hours : 32 Experience : New graduates are welcome;
Skilled Nursing Facility experience preferred License : State License required Certifications : CPR (Fundamental Facilities) Must-Have : COVID-19 test 72 hours prior to start date with results issued 48 hours before starting Job Description : Completion bonus provided upon successful completion of assignment.
- Ensure prescreening is filled out and attached upon application submission.- Submit resume and license verification.- Candidates will be declined if pertinent documentation is missing.
- Conduct state license verification.- Provide a resume and three references.- Complete county criminal background check for the past seven years for all counties lived and worked.
- Undergo federal criminal background check for seven years or more.- Perform a multi-state criminal search for seven years or more.
- Carry out a nationwide sex offender search for seven years or more.- HHS / OIG search is required.- GSA search is necessary.
- Conduct Social Security Number trace.- Provide 1-Step PPD or Quantiferon or proof of positive PPD and chest X-ray.- SAM search is needed.
- Complete Indiana LCH state background check.- Physical examination required.
