Clinical Manager
Low Caseload, Patient Focused : Join the OnCare Health Team* Our Clinical Manager Will : Work closely with the clinical and therapy teams to deliver exceptional home health and hospice care to our patients.
Assist with oversight of the assessment, planning, implementation, and evaluation of patient care.
Through regular review and audit, ensure proper documentation of services and plans of care in compliance with policy and regulatory standards.
Provide direction to intake team regarding home health and hospice eligibility.
Provide direct care team with EMR support and schedule management.
Assist Director of Clinical Services and Director of Therapy Services in the planning, implementation, and evaluation of in-service and continuing education.
Assist with development and maintenance of new programs and processes.
Actively participate in quality assessment performance improvement teams and activities.
Serve in an administrator on-call rotation.
