Job Announcement

Title : Community Health Worker Division : HOPICS

FLSA : 100% Supervisor : Monnie Anderson

Pay Range : $21.00 - $24.00 per hour Revised : 08.20.24

Summary

The knowledge and skills needed for the Community Health Worker position are typically gained through a combination of lived experience with homelessness or incarceration AND navigating associated service delivery systems.

For example : parole, probation, community clinics AND six months experience providing peer support and mentorship to individuals.

Essential Functions

  • Knowledge of community resources in area of residence
  • Comfortable working with diverse populations including LGBT, formerly incarcerated and mentally ill individuals
  • Exceptional ability to connect and engage with people
  • Excellent oral and written communication skills
  • Detail oriented, organized and possess time management skills
  • Able to work flexible job hours
  • Willingness to work in various environments, including jail settings, street outreach, home visits, homeless encampment
  • A history of incarceration or homelessness is desirable
  • Prior experience working with currently and formerly incarcerated individuals, and / or homeless individuals is desirable
  • Prior experience as a health navigator, peer support worker, outreach worker or promotora is desirable
  • Ability to speak Spanish, or other languages is desirable
  • Attendance in all mandatory program and division meetings and trainings, as assigned by his / her supervisor and division director.
  • Maintain appropriate boundaries; and adhere to SSG's Code of Ethics and HOPICS Core Values.
  • Represent the Agency in a professional manner at meetings and community events.
  • Maintain files / records on client services in compliance with HIPAA, 42 CFR Part 2 and other funding requirements for audit purposes.
  • Regular attendance required.
  • Other duties as needed.

Minimum Qualifications - Knowledge, Skills and Abilities Required

Valid CA Class C Driver License or access to reliable transportation to carry out job-related essential functions . High school diploma or GED.

Bachelor's Degree from an accredited college / university desired. Proficiency in Microsoft Word, Excel, and Internet. If in recovery, a minimum of three (3 years of being drug and alcohol free is required.

Neat and professional appearance. Attention to detail and punctuality are required; Ability to communicate effectively, both written and orally required.

TB test, CPR and First Aid certification required. Note : Results may not be more than 3 months prior to or 7 days after date of hire and renewed annually thereafter.)

Supervisory Responsibilities

This position does not have any supervisory responsibilities.

Environmental Conditions (Working Conditions)

This position is responsible for working in "client friendly" environments and may be required to visit other shelters and partner agencies.

There is some responsibility to work in noisy environments where children and unstable adults are few to talk loud and expressive.

Physical Requirements

The staff typically spends time sitting, standing, walking, reading, thinking, note taking, and driving.

Mental Requirements

This position will require the individual to be able to handle any / all the following : constant distractions, interruptions, uncontrollable changes in priorities / work schedules : to be able to process information, think and conceptualize.
