About The Position
Job Title : Rehabilitation Therapy Job Specialty : Occupational Therapist Job Duration : 13 weeks Shift : Monday-Friday, Days Guaranteed Hours : 32 hours per week Experience : Open to new graduates;
experience with Rehab Optima preferred License : Must have valid state license Certifications : CPR certification required Must-Have : - Filled prescreen form- Resume - License verification Job Description : Split work between two facilities.
- Conduct occupational therapy sessions tailored to individual patient needs.- Collaborate with healthcare team to develop and implement patient care plans.
- Maintain detailed and accurate patient records.- Participate in interdisciplinary meetings and provide updates on patient progress.
- Ensure compliance with all regulatory and facility guidelines.- Complete background checks including : - State License Verification - References (3) - County Criminal (7 years for all counties lived and worked) - Federal Criminal (7 years or more) - Multi-State Criminal Search (7 years or more) - Nationwide Sex Offender Search (7 years or more) - HHS / OIG Search - GSA Search - Social Security Number Trace - 1 Step PPD or Quantiferon - SAM Search - State Background - SLED - Hep BPlease ensure all credentialing checklist items are completed prior to application submission, as candidates may be declined if requirements are not met.
