Skin / Wound Ostomy Clinician
About the Job :
As a member of the Patient Care Services team, the Skin / Wound Ostomy Clinician assesses and evaluates the patient’s skin status.
He / She develops age-specific and culturally appropriate plans of care, interventions and evaluations based on assessment of the patient’s needs.
The Skin / Wound / Ostomy Clinician identifies and utilizes age specific teaching techniques appropriate to patient’s and family’s developmental level.
He / She facilitates professional development of clinical staff. He / She demonstrates emerging leadership behaviors through involvement in area activities and performance improvement efforts.
The Skin / Wound / Ostomy Clinician works in collaboration with the healthcare team and is accountable to the Director of the Wound Ostomy Program.
What You’ll Do :
- Responds to referrals to identify patient’s skin, wound, and ostomy needs
- Conducts advanced assessments of patient’s skin, acute and chronic wounds, stomas and fistulas
- Identify modifiable and non-modifiable risk factors so preventative measures can be appropriately implemented.
Who You Are :
- Graduate from an accredited school of nursing. Completion of BSN or higher nursing degree by 2025
- Current NH Licensure as a registered nurse.
- Minimum of three years as a Wound Ostomy Clinician or specialty certification in wound or wound / ostomy care.
- Must possess good communication skills to work with clients, community services, and the health care team.
- Ability to achieve the following competencies : Basic Life Support, Care of the complex ostomate, Management of the complex wound, Vacuum Assisted Closure (VAC) Therapy, Compression Therapy, Mentoring, Must complete a minimum of six hours of continuing education in the management of the skin, wounds and ostomies.
