Position : Care Coordinator
The Care Coordinator will be responsible for providing direct, supportive, and ongoing coordination of therapeutic services to assist individuals with a current or prior SUD diagnosis along with a primary care condition, and their families / natural supports.
Responsibilities include :
- Taking a person-centered approach to developing a team that supports case management, treatment planning, resources identification and assistance.
- Identifying and developing natural supports, advocacy, service monitoring, and follow-up.
- Collaborating with natural supports and other providers.
- Providing individual and group services.
- Serving as a treatment team member and providing backup to other team members as needed.
This position requires a flexible schedule with evening hours both in the office, in the client home, and in the community.
It also participates in the rotation of an after-hours crisis / emergency response including initial crisis assessment, safety risk, and outreach if needed.
What You Will Be Sharing
- Ability to join with, work with, and support individuals and their families with complex mental health and behavioral needs from a strengths-based approach.
- Ability to formulate an individual treatment plan and apply appropriate rehabilitation and treatment modalities.
- Valid Driver’s License including vehicle and appropriate insurance.
Qualifications
- Previous work or volunteer experience with people in crisis.
- Knowledge of community resources.
- Ability to maintain confidentiality of records and information.
- Ability to adapt to changing needs of clients.
- Ability to work with diverse personalities, persons of all ages, ethnicity, religion, and sexual orientation.
- Proficiency in MS Office and excellent administrative skills.
- Strong interpersonal and organizational skills.
- Strong communication skills including verbal and written.
- Proven ability to submit timely, detailed, and accurate case notes.
