Perform home visits with patients enrolled in Evernorth In-Home Primary Care. Initial visits are comprehensive evaluations with subsequent visits targeting risk factors for disease exacerbations and hospitalizations.
Working closely with other team members to ensure quality care is delivered.
Collaborating with the patients' PCPs and health plan case managers.
Educate patients and / or patient's family on chronic medical conditions, preventative care, and medication adherence compliance
Ability to prescribe medications, order labs or diagnostic testing, and make sub-subspecialty referrals collaborating with the community PCP
Review patient records for past and present health issues
Collaborate with the supervising physician and other members of health care team to ensure outstanding patient care is delivered
Role is Work from Home. Candidate must live within the greater Nashville area and our surrounding counties
Candidate will be completing in-home patient visits daily; visit type range from acute, chronic, established, new patient assessment, and annual wellness visits.
This role requires driving responsibilities.
Highly motivated, self-directed professional with strong organizational skills and comfortable working independently on a daily basis
Excellent clinical assessment and analytical skills, as well as efficient problem solving
Great verbal and written communication skills
Ability to recognize conditions, characteristics that place patients at risk, with proven ability to pursue feedback and diagnose and treat acute and chronic diseases
Ability & comfortable working independently in the field, ability to recognize conditions, diagnose and treat both acute and chronic conditions, and EMR experience and proficiency w / various computer applications (Microsoft) is a must
