Responsibilities
Position Summary :
Dignity Home Health Santa Maria provides patient care from our far reaching territories up the coast to Cambria over to Paso Robles and down as far as Santa Maria / Lompoc / Solvang.
We work hard to keep the territory manageable as well as see the patients timely. As a Case Manager you can make an impact in our patients lives and create an inclusive collaborative team model by organizing communicating and aligning a multidisciplinary team.
Our Case Manager RN provides highly skilled nursing care in the home setting as the admitting nurse providing services such as wound care IV medication administration in addition to supervising LVN care delivery and care coordination with the multidisciplinary team.
The Case Manager is the front line clinician communicating with the physician overseeing the plan of care. As a Case Manager your role is integral to leading the case conference and improving clinical outcomes.

Coverage includes but is not limited Santa Maria, Orcutt, Lompoc, Los Alamos, Buellton to Santa Ynez / Solvang. Depending on volume of patients and availability of staffing, coverage may include floating outside of this area as well.
Qualifications
Minimum :
- California Registered Nurse license.
- Current AHA BLS certification
- California driver’s license with proof of liability insurance for automobile.
Preferred :
- One year acute care experience as a RN
- Home Health experience preferred
