Position Summary :
Join our compassionate team as a LPN providing invaluable care to patients in the comfort of their homes needing alternative schedule options.
As a vital member of our homecare agency, you'll play a crucial role in ensuring the well-being and comfort of patients outside regular scheduled hours.
This position offers a unique opportunity to make a meaningful impact on the lives of families by providing high-quality nursing care tailored to the individual needs of each patient.
Responsibilities :
- Supervisory Visits and Education in a Patient's Home.
- Re-evaluation of patients skilled nursing / PSS needs
- Work autonomously with minimal assistance by the Clinical Manager
- Maintain patient confidentiality
- Make decisions based on established policies and procedures
- Preparation of the clinical progress notes.
- Informing the family and Registered Nurse of changes in the patients condition
or needs.
Administration of medications and treatments as prescribed by a physician in
accordance with currently accepted standards of medical practice
- Total patient care
- Patient and family teaching.
- Other services in accordance with and as outlined in the Georgia Licensed
- Professional Nurse Practice Act.
- Reports directly to the Registered Nurse and Clinical Director.
Qualifications :
- Licensed Practical Nurse (LPN) must hold a current unencumbered Nursing license to practice in the state of Georgia.
- Current BLS Certification.
- Current Drivers License
- Current TB Screening
- Current BLS Certification.
- Experience caring for medically fragile individuals.
- Experience in Home Health (preferred).
- Experience in Pediatrics (preferred).
