We are seeking Nurse Practitioners and Physician Assistants with acute care experience to join our team delivering comprehensive health assessments, treatment, and outcome management to patients of all ages.
This role involves providing definitive care in the home, performing minor procedures, point of care laboratory studies, and coordinating timely follow-up care.

Responsibilities :
- Work as part of a clinical care team to deliver care in the patient's home, performing comprehensive health assessments and diagnostic procedures
- Provide therapeutic interventions, such as splinting, wound care, and minor procedures
- Use critical thinking skills and follow evidence-based standards of practice
- Accurately document patient encounters and analyze test data for diagnosis and treatment
- Educate patients and families on acute illness treatment and health management
- Effectively communicate with all individuals involved in the patient's care and proactively solve problems
- Adhere to clinical and safety standards, protocols, and performance metrics
Requirements :
- Minimum of two years of experience as an Advanced Practice Provider
- Graduate with an advanced degree from an accredited institution
- Current unrestricted state license as a Nurse Practitioner or Physician Assistant
- Nationally board certified with current BLS certification, ACLS preferred
- Prescriptive authority and DEA
- Valid driver's license with clean driving record
- Ability to lift and carry equipment up to 50 pounds and work varied schedules, including evenings, holidays, and weekends
