Important things to know about the job :
- Full-time position, 40 hours per week
- 7 : 30 am - 4 pm, Monday through Friday
- Occasional weekends and call

JOB SUMMARY : A professional position where the case manager collaborates with interdisciplinary team members to provide discharge planning services to patients and families.
This position is a part of the Case Management Department, and the primary assignment is the Rehab Unit. QUALIFICATIONS :
- Master’s Degree in Social Work or related field of study from an accredited college program OR Graduate from an accredited school of nursing (BSN degree preferred).
- Minimum 2 years of clinical experience preferred.
- Case management experience preferred.
- Current license to practice in the state of Oklahoma.
- Strong skills in oral and written communication.
PHYSICAL REQUIREMENTS :
- Able to speak clearly and distinctly with staff, physicians, patients, and families.
- Able to extensively utilize telephone for communication.
- Demonstrates adequate physical and emotional stamina to deal with stressful situations and to complete the necessary work schedule.
- Able to sit / stand / walk for extended periods of time throughout the work schedule, with some light lifting and bending.
- Demonstrates adequate vision, or correctable with glasses / contacts, for reading and preparing written documents.
- Able to hear, or adequate hearing with corrections, spoken communications from staff, physicians, patients, and families as well as pages.
- Demonstrates adequate manual dexterity to prepare written documents.
- Able to work in a latex burdened environment.
