Job Summary :

We are an in-home healthcare company seeking a skilled and compassionate Mobile Nurse Practitioner to join our team. Our practice primarily services assisted living facilities, providing care to the geriatric population. As a Nurse Practitioner, you will deliver high-quality patient care in the comfort and familiarity of our clients’ living environments. Working closely with physicians and other healthcare professionals, you will diagnose and treat acute and chronic health conditions, conduct physical examinations, order and interpret diagnostic tests, and prescribe medications. Responsibilities :

• Perform thorough physical examinations on patients in assisted living facilities or their homes and obtain comprehensive medical histories
• Diagnose and treat acute and chronic health conditions and provide appropriate interventions in the facility or home setting
• Order and interpret diagnostic tests, such as lab work and imaging studies, to aid in diagnosis and treatment planning
• Collaborate with physicians and other healthcare professionals to develop and implement personalized care plans for each patient
• Prescribe medications and therapies as needed, considering the unique challenges and requirements of geriatric and facility-based healthcare
• Provide counseling and education to patients, families, and facility staff regarding healthcare management, disease prevention, and lifestyle modifications
• Document patient assessment findings, interventions, and outcomes accurately and in a timely manner
• Maintain accurate and up-to-date medical records, ensuring compliance with medical regulations and standards specific to in-home and assisted living healthcare
• Participate in ongoing professional development activities to enhance your knowledge and skills in caring for the geriatric population
