Our Outpatient Transitional Care program is seeking clinicians (LMHP or LMHP-E) to work with adults with a variety of mental health and / or substance use disorders, as part of our post-crisis services community support.
Our Transitional Care utilizes a supportive team approach including clinical and case management staff.
We offer flexible scheduling, licensure supervision for LPC and LCSW residents and supervisees, and are a National Health Services Corps Loan Repayment site.

Clinician I-Transitional Care : $46,706-Depending on Experience
Clinician II-Transitional Care : $56,518-Depending on Experience
Education Requirements
Clinician I-Transitional Care :
- Master's Degree in Social Work, Counseling or other license-eligible human services field required.
- Required to register and become licensed-eligible within 3 months of hire, with the Virginia Board of Counseling.
Clinician II-Transitional Care :
- Master's Degree in Social Work, Counseling or other license-eligible human services field required.
- State of Virginia license required in Social Work, Marriage and Family or other related field.
