Shrewsbury Nursing & Rehabilitation Center (SNRC) ( http : / / www.shrewsburynursing.com / index.html ) is a family-owned, 99-bed skilled nursing facility located in Shrewsbury, Massachusetts.
We specialize in short-term rehabilitation, long-term care, and memory care. SNRC also maintains a strong palliative care practice as well as respite care and hospice care.
Founded in 1969, SNRC is located on a 30-acre park-like campus which also hosts and is directly adjacent to its sister company, Southgate at Shrewsbury ( http : / / southgateatshrewsbury.
com / ), the premier retirement community of central Massachusetts. In its most recent survey by the Massachusetts Department of Health, SNRC was found to be deficiency-free.
FT MSW Social Worker
DUTIES :
- Complete clinical assessments and social service documentation as required by state and federal mandates and facility policy.
- Demonstrate effective practice skills, which integrate social work theory, regulations and sound working relationships to enhance positive outcomes for patients and families.
- Interviews residents and their family members to solicit information regarding the resident’s strengths, weaknesses, coping strategies and lifestyle prior to placement and develops and initial psychosocial assessment along with a detailed social history within the clinical record,
- Maintains and improves resident’s ability to control everyday physical needs and mental / psychosocial needs
- Identify individual social and emotional needs
- Develops and maintain resident psychosocial assessments and quarterly / routine care plans,
- Educates residents, family members, and / or significant others regarding advance care planning and community resources,
- Works with the interdisciplinary team to assure resident and family concerns are addressed in a timely manner. Advocates for the needs of residents in ensuring resident rights,
- Educate residents, families, and staff on resident rights, resident abuse, mistreatment and neglect regulations to ensure residents are protected.
Participate in orientation process with staff developer.
- Provide information and documentation to residents and families with regards to access to Medicare, MassHealth and financial and insurance benefits.
- Collaborates with social service team members to ensure the needs of residents are met with professionalism, dignity, while providing resident choice.
- Educate resident and families about palliative care and end of life choices.
- Participates in the planning of the resident’s admission, community re-entry, transfer to another facility and discharge planning.
REQUIREMENTS :
Master’s in social work / (MSW)
Long-term care / sub-acute experience required
