Overview :

The Registered Nurse - Case Manager plays a crucial role in providing comprehensive care coordination and case management for patients in a healthcare setting.

They are responsible for ensuring efficient and effective utilization of healthcare resources, advocating for patients' needs, and promoting optimal health outcomes.

Key Responsibilities :

  • Conduct initial and ongoing patient assessments to develop and implement individualized care plans
  • Collaborate with healthcare providers to coordinate and monitor patient care across various settings
  • Facilitate communication and information sharing among the healthcare team, patients, and their families
  • Advocate for patient rights and ensure access to appropriate healthcare services
  • Assess and address patients' psychosocial and support needs
  • Coordinate discharge planning and transition of care for patients
  • Monitor and evaluate patient progress and modify care plans as needed
  • Provide patient education and support for self-management of chronic conditions
  • Document all patient interactions, interventions, and outcomes accurately and in a timely manner
  • Ensure compliance with regulatory requirements and standards of care
  • Participate in quality improvement initiatives and case conferences
  • Conduct utilization review and manage resource allocation for cost-effective care
  • Collaborate with community resources to support patients' post-discharge needs
  • Participate in interdisciplinary team meetings and contribute to care planning

Required Qualifications :

  • Active Registered Nurse (RN) license
  • Bachelor of Science in Nursing (BSN) or equivalent
  • 5+ years of clinical nursing experience, preferably in case management or care coordination
  • Knowledge of healthcare delivery systems and regulatory requirements
  • Strong critical thinking and decision-making skills
  • Excellent communication and interpersonal abilities
  • Demonstrated skills in patient advocacy and empowerment
  • Proficiency in electronic health records and care management software
  • Ability to work effectively in a multidisciplinary team
  • Strong organizational and time management skills
  • Understanding of healthcare quality and utilization metrics
  • Commitment to ethical and professional conduct
  • Ability to adapt to changing healthcare environments and patient needs
  • Certification in case management (CCM) is preferred
  • Current Basic Life Support (BLS) certification
