Overview :
The Registered Nurse - Case Manager plays a crucial role in providing comprehensive care coordination and case management for patients in a healthcare setting.
They are responsible for ensuring efficient and effective utilization of healthcare resources, advocating for patients' needs, and promoting optimal health outcomes.
Key Responsibilities :
- Conduct initial and ongoing patient assessments to develop and implement individualized care plans
- Collaborate with healthcare providers to coordinate and monitor patient care across various settings
- Facilitate communication and information sharing among the healthcare team, patients, and their families
- Advocate for patient rights and ensure access to appropriate healthcare services
- Assess and address patients' psychosocial and support needs
- Coordinate discharge planning and transition of care for patients
- Monitor and evaluate patient progress and modify care plans as needed
- Provide patient education and support for self-management of chronic conditions
- Document all patient interactions, interventions, and outcomes accurately and in a timely manner
- Ensure compliance with regulatory requirements and standards of care
- Participate in quality improvement initiatives and case conferences
- Conduct utilization review and manage resource allocation for cost-effective care
- Collaborate with community resources to support patients' post-discharge needs
- Participate in interdisciplinary team meetings and contribute to care planning
Required Qualifications :
- Active Registered Nurse (RN) license
- Bachelor of Science in Nursing (BSN) or equivalent
- 5+ years of clinical nursing experience, preferably in case management or care coordination
- Knowledge of healthcare delivery systems and regulatory requirements
- Strong critical thinking and decision-making skills
- Excellent communication and interpersonal abilities
- Demonstrated skills in patient advocacy and empowerment
- Proficiency in electronic health records and care management software
- Ability to work effectively in a multidisciplinary team
- Strong organizational and time management skills
- Understanding of healthcare quality and utilization metrics
- Commitment to ethical and professional conduct
- Ability to adapt to changing healthcare environments and patient needs
- Certification in case management (CCM) is preferred
- Current Basic Life Support (BLS) certification
