Job Description
The Nurse Practitioner (NP) is a skilled member of the health team who provides direct preventive and primary care for pediatric, adults and geriatric patients under collaboration with a licensed physician, according to Nurse Practitioner Protocol, and within the scope of the Nurse Practitioner License in the State of Wisconsin.
Family Health Center provides comprehensive, integrated primary health care services for all, regardless of one’s background or circumstances.
Care provided is related to health maintenance, acute care, and chronic disease management at the health center or in outreach settings including mobile services.
JOB QUALIFICATIONS
For positions requiring education beyond a high school diploma or equivalent, educational qualifications must be from an institution whose accreditation is recognized by the Council for Higher Education and Accreditation.
Minimum Required : Master’s Degree in Nursing.
Preferred / Optional : None
EXPERIENCE
Preferred / Optional : Three to five years’ nursing practitioner experience.
CERTIFICATIONS / LICENSES
The following licensure(s), certification(s), registration(s), etc., are required for this position. Licenses with restrictions are subject to review to determine if restrictions are substantially related to the position.
Minimum Required : Current State of Wisconsin Nurse License or Nurse Compact License, and Nurse Practitioner Licensure. Current Drug Enforcement Administration (DEA) License.
Mobile Care : Basic Life Support (BLS) certification awarded by the American Heart Association (AHA). Or clinic : Advanced Cardiac Life Support (ACLS) awarded by the American Health Association (AHA) at time of hire.
DATA-waiver prescription privilege (DEAX). Additional department-specific certifications, licenses, or memberships may be required (varies by department / location).
